How to use the Flowsheet


Dear Colleague,

This is the
Canadian Centre for Research on Diabetes Flow Sheet intended to assist primary care practitioners to treat their Diabetic patients according to CDA Guidelines.

You may use this as a stand alone tool or
• as a
Flowsheet with feedback from our Clinic; our special tool for Guideline Directed Care to help better patient outcomes.

To obtain our feedback, print the flowsheet and with the consent of your patient*, fill in the patient information and fax it to the
Canadian Centre for Research on Diabetes at fax (613) 283-9020 or Toll Free Fax 1-866-696-4099. We then calculate BMI, Total Cholesterol/HDL ratio and Creatinine Clearence and mail you a new Flow Sheet in colour with all values exceeding CDA Guideline targets highlighted in red. If applicable to your patient, you will also find CDA Guideline Recommendations at the bottom of the Flowsheet. You may keep the Flowsheet in the patient chart for their next visit.

Once again at their next visit, you make your treatment decisions, and enter any medication changes, new biometric values and fax back to us, so we update your patient’s information and send you a new Flow Sheet. And so on.

Periodically you may receive reports on your own practice compared to your peers. These reports are confidential and can be sent only to you.

* Both your identification and all patient identifiers are removed and aggregate data on all patients in Canada is published.
