How is ADP funding calculated for the supplies grant?

The ADP provides funding assistance of $2400 per year, divided into four payments of $600 every three months. The first payment will include funding assistance from the date insulin pump therapy was initiated until the date the ADP receives the application form. For ongoing payments, $600 will be sent every three months.

If pump therapy was initiated before April 1, 2006, the first payment will include funding assistance from April 1, 2006 until the date the ADP receives the application form. For ongoing payments, $600 will be sent every three months.

If pump therapy was initiated after April 1, 2006, the first payment will include funding assistance from that date onward until the date the ADP receives the application form. For ongoing payments, $600 will be sent every three months.

How are retroactive pump payments made?

If the applicant purchased a pump between April 1, 2006 – November 30, 2006 and is able to provide proof of payment along with the retroactive pump application form, the ADP will reimburse to the client their portion paid, to a maximum of $6300

NOTE - An Application for Retroactive Funding of Insulin Pumps for Children form and the Application for Funding Insulin Pumps and Supplies for Children form must be completed and submitted to the ADP together at the same time.

Do application forms expire?

Application forms become ‘stale dated’ when the physician’s signature date is greater than one year from the date the ADP receives the application. A stale dated application cannot be processed.

Does the physician’s date in Section 2, the vendor’s date in Section 3 and the date of delivery in Section 4 of the Application for Funding Insulin Pumps and Supplies for Children form have to be signed in any particular date order?

No. There are no date restrictions placed on the physician’s date, vendor’s date or the delivery date with respect to their date order.

Are faxed or photocopied versions of the Application for Funding Insulin Pumps and Supplies for Children form acceptable?

No. The application must have original signatures in order to be processed.

What if a client is no longer followed by a Paediatric Diabetes Program but is still under the age of 19 and requires and insulin pump and related supplies?

A client may obtain a Special Authorization Form from the ADP by contacting the Program Coordinator at 416-327-8178 or 1-800-268-6021 x 78178.

What if a client is geographically unable to get to an ADP-registered Paediatric Diabetes Program?

A client may obtain a Special Authorization Form from the ADP by contacting the Program Coordinator at 416-327-8178 or 1-800-268-6021 x 78178.

Should the Paediatric Diabetes Program keep copies of the ADP application form?

Yes. A copy of each application sent to the ADP must be kept in the client’s file.

What if information is missing on an application form sent in to the ADP?

The application form must be completed in its entirety. The ADP will return applications with missing information to the paediatric diabetes program for completion which will result in processing delays.

What date does the ADP use to determine the 5 year replacement date for the pump?

The physician’s date found in Section 3 of the Application for Funding Insulin Pumps and Supplies for Children form.

What is the ADP Registered Vendor’s role with respect to the trial period?

The vendor must provide assessment and trial equipment requested by the ADP Registered Paediatric Diabetes Program. The ADP does not specify the timeline involved or cost to the client during the trial period. It is recommended that the vendor specify timeline and any associated client costs before dispensing any trial equipment.
